The Center for Healing and Wholeness Resource Consultant, Jeanne Schuller works one on one with seniors and their family caregivers to help them navigate resources and plan for their future care needs by providing the following levels of support.
She can help provide information and support in areas such as: senior housing options, homemaking and companion services, transportation resources, home health care agencies, support groups, legal and financial services, and assistance for family caregivers . This is usually a single request and one time service. Generally, Jeanne responds to a client request for information or list of services by phone/e-mail or in writing.
This usually involves meeting in person with the elder and family members or spouse. These meetings involve conversations about needs or goals, include a brief assessment, and result in the creation of a written Action Plan. Sometimes, additional family members are involved. Goals are prioritized and pursued with the support of family, friends, medical or other paid service professionals. Jeanne serves in a supportive and coaching role to the elder and family caregivers. This tier is more involved and may include 2-3 in person contacts.
Jeanne received her Masters in Social Work from the University of Saint Thomas and Saint Catherine University. She is a licensed graduate social worker with the state of Minnesota. She has experience doing care planning with older adults and their caregivers, facilitating support groups and consulting on various Alzheimer’s disease/dementia related health care initiatives. She also co-teaches the graduate level course, Social Work Practice with Older Adults and Their Families at St. Catherine University and the University of Saint Thomas.
